Clarifying Your Wishes: An Overview & Tools for Medical Advance Care Planning – Virtual
Have you thought about who would speak on your behalf if you were not able to voice your own healthcare decisions, and would they know your values for care? Have you been encouraged to complete medical advance directives (MDPoA, Living Will, DNR, or MOST form) and are confused about what these letters mean or where to start?
This one-hour informational session will:
Give an overview of medical advance care planning (ACP)
Offer tools and resources to help you clarify your values and feel more prepared to talk with your medical team and loved ones about your wishes
Inform you about documents you can use to specify your wishes for medical care
This session is open to patients, survivors, and caregivers. ACP can be of value for everyone 18 and older, regardless of illness status, so it is never too early to start!
